Forms

We provide a variety of forms for you to be able to do business with EIB including:

• Applications: Dozens of applications are available (call us directly if you can't find what you need).
• Producer Agreement: Before EIB can place business for a producer, a producer agreement must be completed, submitted and accepted by EIB.
• Risk Management: To assist insureds, we provide risk management services for which information gathering is key. We provide various forms online for your reference.
• Claims: Claims are inevitable and require reporting. Forms to ensure proper reporting are provided for your use in reporting claims.
• Administrative: Special forms are required under certain circumstances, many of which we have made available online for your convenience.
• Financing: Prime provides a financing option for insureds.
